Su objetivo es dotar a las personas mayores de estrategias y habilidades que promuevan la autogestión de su autocuidado y el mantenimiento de la salud a través de hábitos saludables, que les permitan gestionar su salud física de la forma más independiente posible, en coordinación con su familia o persona de referencia, utilizando un enfoque centrado en la persona que tenga en cuenta el contexto en el que vive.

Aim of the Good Practice
This good practice has the following objectives: To identify risk factors that may contribute to the deterioration of the older person's physical health. To promote healthy habits in physical activity, nutrition, avoidance of harmful substances, sleep routines, hygiene practices, and medical care. To provide support and tools that enable the person to manage their health habits, helping them preserve their abilities and maintain independence in caring for their health.
Target Group of the Good Practice
The profile of the individuals served at the day center includes older adults over the age of 60 with dependency issues who have chronic illnesses, multiple pathologies, and limited autonomy. They require specialized social and health care, medical supervision, nursing care, or rehabilitation services that cannot be adequately provided at home.

Consistency over time
A follow-up evaluation of the individual goals outlined in the Individual Support Plan (ISP) is carried out periodically (at least every six months), using the same assessment tool applied during the initial evaluation, following a pre-post intervention approach. Based on this evaluation, the continuation or adjustment of the goals is determined.
Evaluation of the Good Practice
An evaluation is conducted using a healthy habits inventory to assess the individual's habits in various health-related activities, specifically in terms of physical inactivity, nutrition, substance use, sleep, hygiene, and health care. Data collection is carried out through direct observation, scheduled evaluations, and interviews with the individual and their family or reference person.
