The Prisma 7 project aims to lead active surveillance, to detect any situations of functional difficulties in the elder population unknown to the territorial social welfare system, in order to reduce and/or delay the risk of disability, to institutionalise as well as to promote a better quality of life in people.
The tool uses is “Prisma 7” survey – Research Program on the Integration of Services for the Maintenance of Autonomy, that consists of seven questions, developed by university research groups of the St-Augustin de Beauport Center in Québec (Canada) and by the Research Center on Aging of the Sherbrooke University Institute of Geriatrics.

- The aim is to drive policies toward meeting real needs, to prevent a worsening of the frailty state and to delay EMPLOYMENT as much as possible, promoting physical and social well-being aging. The project includes two phases: 1.the screening phase to learn about and map fragility 2.the surveillance/early care phase of people recognized as potentially frail, who are going to be contacted by their health regional unit to define a wellness project.
- The target population is elderly aged 75 years older.
- The project started in 2018 in 4 pilot regional Health Districts. From 2019 to today. the project was extendend to all the Districts.
- Surveillance takes place regularly.
- The best Practice has been monitored, as well as the approval of the elderly population. We are going to develop the impact evaluation now.
