The Prisma 7 project aims to lead active surveillance, to detect any situations of functional difficulties in the elder population unknown to the territorial social welfare system, in order to reduce and/or delay the risk of disability, to institutionalise as well as to promote a better quality of life in people.
The tool uses is “Prisma 7” survey – Research Program on the Integration of Services for the Maintenance of Autonomy, that consists of seven questions, developed by university research groups of the St-Augustin de Beauport Center in Québec (Canada) and by the Research Center on Aging of the Sherbrooke University Institute of Geriatrics.
